The following sections apply when you register for, participate in or agree to serve as a volunteer with Baptists on Mission, North Carolina Baptist Men, Inc., or any related mission project, disaster response project, rebuild project, mission trip, training, event or volunteer activity.
By agreeing to this Volunteer Medical Authorization and Liability Release, you acknowledge that you have read, understand and voluntarily agree to the terms below.
Your privacy is important to us. During volunteer registration or participation, BOM may request or receive health-related information, emergency contact information, allergy information, medication information or other information that may be helpful in the event of an emergency.
In a medical emergency, it may be necessary for BOM to disclose relevant health or emergency information to a medical provider, emergency responder, hospital, event leader, volunteer coordinator, or other appropriate third party involved in providing care, safety assistance or emergency response.
By agreeing to this authorization, I authorize BOM to use and disclose the health-related information I provide as reasonably necessary for volunteer coordination, emergency response, medical care, safety, risk management and related administrative purposes.
I understand that this authorization is voluntary. I also understand that information disclosed to a third party may be subject to re-disclosure by that third party and may no longer be protected by BOM.
I understand that I may revoke this authorization by providing written notice to BOM, except to the extent that BOM has already acted in reliance on this authorization or as otherwise permitted by law.
This authorization will remain in effect until revoked by me in writing or until it is no longer needed for the volunteer, registration, event, trip, account or ministry-related purpose for which it was provided.
I confirm that I have read this authorization and that it is consistent with my directions. By agreeing, I authorize the use and disclosure of my health-related information as described above.
Please read this section carefully. This section is part of your agreement as a volunteer and describes your understanding of your working relationship as a volunteer with Baptists on Mission and North Carolina Baptist Men, Inc.
The work of short-term volunteer missionaries and disaster response volunteers may involve risk and danger to personal safety. These risks may include, but are not limited to, physical injury, accidents, medical emergencies, illness, exposure to hazardous conditions, criminal activity, unsafe structures, transportation risks, weather conditions, natural disasters, emotional stress, physical exertion and other unexpected circumstances.
By participating as a volunteer, I acknowledge and agree to the following:
I have chosen to participate in this mission project, mission trip, disaster response project, rebuild project, training, event or other volunteer activity voluntarily.
I understand that this work may be hazardous and may be physically, mentally and emotionally demanding.
I understand the potential risks involved and certify that I am in good health and physically able to participate in the volunteer activity for which I have registered.
I understand that I am participating at my own risk. I assume all risk and responsibility for any personal injury, illness, property damage, property loss, medical cost or other expense that I may sustain or incur while participating in this volunteer activity, including travel, lodging and related activities.
I understand that BOM is not responsible for my personal property or belongings. I agree to hold BOM harmless in the event of theft, damage or loss of personal property from any source or cause.
I agree to follow all rules, safety instructions, policies, procedures and directions provided by BOM, project leaders, event leaders, volunteer coordinators, host churches, ministry partners or other authorized representatives.
I understand that failure to follow rules, safety instructions or leader directions may result in removal from the volunteer activity.
For myself, my estate, my heirs and my personal representatives, I release, discharge, indemnify and hold harmless North Carolina Baptist Men, Inc., doing business as Baptists on Mission, BOM, the local Baptist church, my Baptist association, the Baptist State Convention of North Carolina, ministry partners, host sites, project sites, and their respective officers, directors, employees, agents, servants, volunteers and representatives from any and all claims, demands, losses, damages, causes of action, liabilities, costs or expenses arising from or related to my participation in this volunteer activity, including travel, lodging and related activities.
I understand that this waiver, release and indemnity agreement is fully understood by me and that I enter into it willingly.
I understand that this release will remain in effect for the volunteer activity for which I am registering and, where applicable, for future volunteer activities with BOM unless I provide written notice to BOM or unless a new agreement is required.
By agreeing, I acknowledge that I have read this Volunteer Medical Authorization and Liability Release, understand its terms, and agree to be bound by it.
